The Annual Church Profile (ACP) is a simple, 5-minute form that helps N.C. Baptists see how God is moving across our state. Completing it ensures your church’s story is counted and strengthens the ways we serve you.
Each January, North Carolina Baptist churches are invited to complete the Annual Church Profile, a quick 5-minute digital form that helps tell the story of how God is at work across our state.
The ACP is a tool that strengthens partnerships, provides relevant resources and celebrates ministry fruit across our state convention. Submitting your ACP helps N.C. Baptists know where churches are thriving, where support might be needed and how to connect churches with resources such as training, church revitalization assistance, mission opportunities and more.
The 2025 ACP opens Jan. 1, 2026 ,and asks for a full 12 months of data covering Jan. 1-Dec. 31, 2025. If your church uses a fiscal year, report the most recent full 12-month period that represents your completed year. The form is online at ncbaptist.org/acp and can be saved and returned to. Each church can submit one form.
The digital form eliminates paper, lets you start and finish at your convenience and can be shared easily with staff or volunteers who maintain your church records.
Make reporting simple
- When: The ACP opens Jan. 1 and covers the prior calendar year from January to December. If your church closes on a fiscal calendar, report the most recent completed 12-month period.
- Who completes it: Typically the pastor, church administrator or clerk — whoever has access to attendance, membership and giving records.
- How it works: Complete the short online form. You can save progress and return later. Submit one form per church.

3 reasons to complete the ACP this January
- Celebrate together. Your church’s ministry efforts and testimonies, like baptisms, small groups and missions projects, get counted and celebrated across the state.
- Access better resources. ACP data guides where training, revitalization support, church-planting help, catalyst visits and other resources are most needed.
- Shape partnership and mission. Completing the ACP ensures your voice is heard when messengers gather and budgets and initiatives are planned.
